The NHS across County Durham and the Tees Valley are making changes to the way you can order your repeat prescription.

Your community pharmacy will no longer be able to order your repeat medication, but you will be able to order your repeat medication directly with the practice, using the repeat ordering slip or online.

There is nothing to worry about - you will still receive the medication that you need.

If you order your repeat prescription directly from the GP practice this change will not affect you.

These changes will take effect from Monday 2nd September 2019.

Please ensure you order your repeat prescriptions when you have about 7 days of medication left and only order the medicines you need.

If you still want your community pharmacy to collect and deliver your prescription, please ensure you inform your chosen pharmacy.

To save time, you may print off the Registration Form below, complete it and bring it with you to the practice to register, along with 2 forms of identification. Please do not sign the form until you bring it into the surgery with your identification.

When you register you will also be asked to fill out a medical questionnaire.  This is because it may take a considerable time for us to receive your medical records.  There is a version below which you can print out and complete then bring with you when you come for your new patient hPreRegistrationealth check.
